WJ Baseball Liability Waiver
As parent/legal guardian of the player named on this document, I understand, recognize, and assume that participation in this sport may cause bodily and/or emotional injury to my child. I authorize West Jeff Junior Baseball and its representatives to act on my behalf in accordance with its best judgment in case of emergency. I recognize that any public entity or private facility on which the games are played, or practices are held, the staff, coaches and Directors of West Jeff Junior Baseball, the other players and their parents/legal guardians associated with West Jeff Junior Baseball are not held liable for any accident or injury to myself and/or my child(ren) incurred while participating in the normal activities associated with this sport, either in league games, practices, or other sanctioned events. I also recognize this waiver extends to the coaches and Directors and to the players and their parents/legal guardians from other teams in other leagues, its staff, coaches and Directors are not held responsible for reimbursement of medical expenses resulting from said injury(s).
West Jeff Junior Baseball League maintains a 100% refund policy less a $25 processing fee for cancellation of a registration prior to formation of teams. West Jeff Junior Baseball must receive the cancellation in writing at the league offices, P.O. Box 1463, Evergreen, CO 80437. I acknowledge that telephone notification is not sufficient, and that no refund will be processed until a request is received in writing. After the official formation of teams, but prior to the start of league games, any refunds will be 50% of the original registration costs. These fees will be refunded after the return of any and all West Jeff Junior Baseball Equipment by the participant. This includes but is not limited to: catchers gear, and other equipment the participant may have borrowed. After the start of league games, there will be no refunds.
I acknowledge the financial responsibility of my portion of the league's capital investment in baseball equipment. In the event the team and its representatives do not return any or all of the assigned equipment, I understand I will be billed my pro-rata share of the replacement costs. I also understand that my child(ren) will not be allowed to participate in future sessions until this amount has been paid. I also understand the amount on the front of this form under the field labeled "Total Due With Registration" represents only the initial registration fee. After my child has been placed on a team, any balance will be due and payable, and additional equipment, including uniforms, will not be issued until this amount is paid. The total registration fee for each child is detailed in the newsletter, by age and ability level. I understand this amount is based upon number of games and different league costs.
